Below are some common forms for our patients, and some answers to frequently asked insurance questions and general questions. If you need additional information, please let us know.

Form

This form must be filled out by all new patients. Please print, fill out and bring in 15 minutes prior to your visit or alternately, FAX your completed form to our office one week before your appointment.

Insurance Information

Our office does not contract with any insurance plans. We are classified as an “out-of-network” physician with your insurance company. Most PPO insurance companies will reimburse you for a portion of your claim to an “out-of-network” doctor. While we do require full payment at the time of service, we will provide a statement that you can mail directly to your insurance company for reimbursement upon request. Most insurance companies will send you a check within 2-3 weeks if your procedure is covered. Medicare will not reimburse claims from our office, but if you have secondary insurance, they may provide reimbursement.

Follow-up Visits

Annual Skin Exam is 25 minutes – the visit code for your insurance provider is 99214.

  • 15 minutes – the visit code is 99213
  • 10 minutes – the visit code is 99212
Initial Visits

If you are a new patient please fill out the New Patient Registration form and send it to our office at least 3 days prior to your appointment. You can find this form here or we can mail, email or fax it to you.

At your first visit, please bring copies of any skin biopsy results, recent laboratory tests and a list of all products (over-the-counter and prescription) that have been used for the condition to be discussed.

We will provide a gown for your full body skin check or you can wear your swim suit or trunks.

Appointments

How long in advance do I need to make an appointment?
  • Full Skin Exam we recommend booking at least 3-6 months in advance.
  • Medical Dermatology Appointment 2-4 weeks in advance 
  • Cosmetic Dermatology Treatments we recommend booking 1-2 weeks in advance 
  • Aesthetic Treatments we recommend booking 1-2 weeks in advance   

We are not doing the refresh society at this time

What if I need to cancel or reschedule my appointment?

We understand that everyone is busy and that schedules can change. Our office is booked to ensure that we can provide as many quality appointments as possible in a given day. However, if you miss an appointment or cancel any appointment with less than 24 hours notice, you will be billed a $50 cancellation fee and be required to leave a credit card number to re-book another appointment. If you find that you cannot make a scheduled appointment please give us as much notice as possible. This will allow us to bring in other patients who have been waiting for appointments.

How long do appointments last?

New Patient full skin checks are up to 40 minutes.

First time Focused Visit and Annual Skin Checks are up to 25 minutes.

What does “Board Certified Dermatologist” mean?

To be Board Certified, the doctor must have completed a three-year residency and passed a rigorous two-part test administered by the American Board of Dermatology. When you see a Board Certified Dermatologist you will be treated by a medical doctor who has a passion for treatment of the skin. They are highly trained in the health of skin, hair and nails.

Why should I see a “Board Certified Dermatologist”?

A Board Certified Dermatologist is a highly-trained medical doctor with experience and passion for keeping skin, hair and nails healthy for your lifetime. Your First Visit

When should I arrive for my appointment?

Please arrive a couple of minutes before your scheduled time. Dr. Scheel tries her best to stay on schedule and spend the most time with you as possible.

Types of Visits

There are many types of dermatological visits but, in general, your first visit to our office will be either a “New Patient Visit” or a “Focused visit.”

  • New Patient Visit: A 45 minute appointment with a thorough assessment of your medical and skin history, a full body skin check and a written treatment plan for your skin care.
  • Focused Visit: A 30 minute visit to our office for a specific skin condition. Examples of a focused visit are visits to check spots on your skin for skin cancer, diagnose a rash, treat acne orinfections, and look at hair loss, nail problems, itching, boils, and warts.
  • Annual Full Skin Exam: A 25 minute visit to our office for your yearly full skin checkup.
  • Follow Up Visit: A 15-30 minute visit to re-check your skin condition, rash, provide a resolution of your pre-cancers or check progress of our treatment.
  • Nurse Visit: A 10-45 minute visit with one of our nurses.
